Out-of-Network and Self-Pay Options
If your insurance plan is *not in-network, we can provide a **superbill* that you may submit to your insurance company for potential out-of-network reimbursement. Reimbursement rates vary by plan and may range from 0–100%. We recommend contacting your insurance provider directly for details on your specific coverage.
All self-pay appointment fees are due at the time of service.
Self-Pay / Cash Rates
- Initial Consultation: * $300 for a 60-minute visit (prepaid rate). The full billed rate, if not prepaid, is typically $400 depending on the complexity and length of the visit.
- Follow-Up Appointment: $150 for a 30-minute visit (prepaid rate). The full billed rate, if not prepaid, is typically $250 depending on the complexity and length of the visit.
- Letters or forms: Letters or forms completed during a session are included; more extensive documentation outside of appointments will be billed at $40 per document/letter. Some documentation may require extensive time or complexity and may justify a higher fee, which will be disclosed to you prior to preparing the documents. - Injectable Medication Administration: * $40 per administration.
Important Notes
- Claims submitted to insurance are billed at the contracted insurance rate, which may differ from the self-pay rate.
- Patients must elect *either self-pay or insurance billing* as both options cannot be used for the same appointment.
- Insurance verification provides only an estimate of coverage; your *Explanation of Benefits (EOB)* from your insurer is the final determination of costs and coverage
- Discount: The practice offers discounts of 10% to all patients over 65 years of age, full-time students, full-time teachers, first responders, nurses, ministers, active-duty military, and veterans.
